All Other Requests

Please use the following form to obtain a copy of an existing Police Report.  This form is submitted to the Brisbane Police Department, Records Division. 

Temporary procedures for requesting and releasing of reports due to COVID-19:

• Completed forms must be accompanied by a copy of the requestor’s driver’s license and emailed to PDRecords@brisbaneca.org or faxed or mailed (using address: 50 Park Place, Brisbane, CA 94005).  We’re trying to avoid in-person contact and email is preferred since staff is also working remotely. 
• Approved report copies will be mailed only (we’re unable to email reports due to security reasons)
